NOTE: The deadline for submitting required documents has been extended for an additional month. For example,

• The Sponsorship Agreement Form is now due by Nov. 15 2008.
• Payment is now due by Dec. 31 2008.
• The Award Nomination Form and company information are now due by Nov. 30 2008.
